Accident Investigation
We investigate accidents, injuries and near misses in regard to workplace situations and public places. The purpose of our Investigations are to assist organizations an to identify why a Situation occurred and what needs to change to prevent similar situations occur in the future.
The reports we prepare for legal practitioners are used by defence firms on the issues of negligence actions or omissions. We also prepare reports as part of the defense process and prosecutions under the Workplace Health & Safety Act & Regulations.
Investigative
In investigating incidents internally for organizations we aim to identify the factors that contributed to the incident and detail controls measures needed to prevent a recurrence. Our analysis identifies the range of factors that contributed to the incident; the types of factors we examine are:
- Person factors - training, experience, activities, supervision, etc.
- Equipment factors - suitability, condition, design, etc.
- Environmental factors - weather, work conditions, temperature, lighting, etc.
- Procedural factors - instructions, work practices, systems of work, etc.
We also train persons within organization on how to investigate incidents objectively to reduce the possibility of similar incidents occurring again.
Expert Witness Reports
We investigate a range of injuries for both Defendant and Plaintiff matters in regard to personal injury matters. The reports we prepare have been used to assist in establishing liability or helping bring the matters to settlement. As an Expert Witness, our investigators have years of experience in this area and have prepared numerous reports. Reports have covered a wide range of injuries, both at work and in public places.
